The question on the ballot:

A replacement of a tax for the benefit of the Appleseed Joint Ambulance District of Hancock County, Ohio for the purpose of PROVIDING AMBULANCE SERVICE, EMERGENCY MEDICAL SERVICE, OR BOTH at a rate not exceeding 0.6 mill for each one dollar of valuation, which amounts to six cents ($0.06) for each one hundred dollars of valuation, for a period of five (5) years, commencing in 2012,first due in the calendar year 2013.[3]
