How to request a block

Hey, I don't know exactly how to request a block but user David_W_Bowen has contributed only vandalism to ballotpedia. His user contrib page should be sufficient to block him. Will4274 12:18, 26 September 2012 (CDT)

Thanks for the fixes. So, no space between TEXT and ref tag. Got it. I've had problems with using < ref refname=xyz >, such that the entire paragraph containing it does not render.

Appreciate guidance/reference on this (the wikipedia markup page failed to illuminate).
