This measure sought to add to the current street levy by a rate of $.20 per $100 of assessed value for a continuing period of time in order to further pay for street improvements as needed in the city.[2]

The question on the ballot:
| An additional tax for the benefit of the City of New Carlisle for the purpose of repairing, improving and replacing streets at a rate not exceeding 2 mills for each one dollar of valuation, which amounts to $0.20 for each one hundred dollars of valuation, for a continuing period of time, commencing in 2012, first due in calendar year 2013. |
