A Maine Form of Ballots Initiative, appearing on the ballot as Measure 1, was on the June 19, 1972 ballot in Maine. It was a citizen-initiated measure to change the form of ballot from party column to office type. It passed.

- Opposed: 64,506
The Act was later amended and revised by the state legislature.
It was the tenth initiative to appear on the Maine ballot subsequent to the time that I&R was approved as a process in Maine in 1908.
