Four statewide ballot questions were on the November 3, 1914 ballot in Ohio.

All four measures on the ballot were initiated constitutional amendments. It was the second year that citizen-initiated measures were on the Ohio ballot. Issue 1 from 1914 was the first citizen-initiated measure to be approved by the state's voters.
