The question on the ballot:

Shall the Ordinance providing for a 1.65% levy on income for the purposes of general municipal operations, maintenance, new equipment, extension and enlargement of municipal services and facilities, capital improvements and retirement of notes and bonded indebtedness of the Village of Ottawa be passed?[3]
