West Virginia voters say machines switched their votes
From Ballotpedia

Electronic voting machines in at least two West Virginia counties are switching some voters' selections. The clerks in the two affected counties, both Republicans, say there isn't a problem. But so far eight voters have said publicly that the machines switched their votes--generally from Obama to McCain.[1]
Machines' manufacturer banned in California
Election Systems and Software, the company that manufactures the machines used in West Virginia, is no longer used in California because of alleged problems with accuracy. But the county clerks in West Virginia defended the machines. Jeff Waybright, the clerk in Jackson County, said he suspected that the problem was with voters who mistakenly chose the wrong candidate. Putnam County Clerk Brian Wood said that he supports the use of the machines, but that no system is without possible flaws.[1]
