A Portage County Health District Tax Levy Replacement question was on the November 6, 2012 election ballot in Portage County, which is in Ohio, where it was defeated.

This question authorized the Portage County Health District to 0.4 mill ($0.4 per $1,000 of assessed valuation) for five years in order to fund the Current Expenses.[1]
