A Columbiana City EMS Tax Levy Renewal question was on the November 6, 2012 election ballot in Mahoning County, which is in Ohio, where it was approved.

This question authorized the city of Columbiana to renew a property tax levy of 2 mills ($2 per $1,000 of assessed valuation) for five years in order to fund ambulance and emergency medical services.[1]
