The Florida Medicaid Expansion Initiative (Initiative #18-16) may appear on the ballot in Florida as an initiated constitutional amendment on November 7, 2028.

The ballot initiative would require the state to expand Medicaid coverage to individuals age 18-65 with incomes at or below 138% of the federal poverty level.[1]

Requires State to provide Medicaid coverage to individuals over age 18 and under age 65 whose incomes are at or below 138 percent of the federal poverty level and meet other nonfinancial eligibility requirements, with no greater burdens placed on eligibility, enrollment, or benefits for these newly eligible individuals compared to other Medicaid beneficiaries. Directs Agency for Health Care Administration to implement the initiative by maximizing federal financial participation for newly eligible individuals.[2]

The state process

An initiated constitutional amendment is a citizen-initiated ballot measure that amends a state's constitution. Eighteen (18) states allow citizens to initiate constitutional amendments.

In Florida, the number of signatures required for an initiated constitutional amendment is equal to 8% of the votes cast in the last presidential election. Proponents must obtain signatures equaling at least 8% of the district-wide vote in the most recent presidential election in at least half (14) of the state's 27 congressional districts. A 60% vote is required for voter approval. In 2006, voters passed an amendment for the 60% vote requirement.


Details about the initiative

  • Florida Decides Healthcare filed the initiative. The initiative was approved for signature gathering on December 12, 2018. Sponsors announced on February 1, 2024, that they would attempt to gather enough valid signatures to place the initiative on the 2026 ballot.[1][3]
  • On September 25, 2025, the campaign announced it would be targeting the 2028 ballot.[4]
