The question on the ballot:
| A renewal of a tax for the benefit of the Four County Board of Alcohol, Drug Addiction and Mental Health Services District (Defiance, Fulton, Henry and Williams Counties) for the purpose of CURRENT EXPENSES FOR PROVIDING MENTAL HEALTH SERVICES at a rate not exceeding 0.7 mill for each one dollar of valuation, which amounts to $0.07 for each one hundred dollars of valuation, for five years, commencing in 2012, first due in calendar year 2013.[6][7] |
