Local Candidate Survey 2021
Candidates running for local office outside of the 100 largest cities or a state capital can use this form to submit your biographical information, a photo, and responses to questions about your political philosophy. Beyond the basic required information, noted with an asterisk, the remaining questions are optional. We recommend choosing at least 4-7 additional questions to answer. The questionnaire takes between 45 and 50 minutes to complete; you can save and resume later if you need to.

Candidates who complete the survey will have a page created specifically for them that includes their responses. This will not be connected to an election page, as these elections are currently out of Ballotpedia’s coverage scope. After the election, these pages will be removed from Ballotpedia.
