A Maple Heights City Fire Levy Renewal measure was on the November 6, 2012 election ballot in Cuyahoga County, which is in Ohio where it was approved.
This measure renewed an up to 2 mills for each $1 of valuation (20 cents for each $100 assessed valuation) for five years to pay salaries, maintenance and operational costs for the Division of Fire.[1]

Language on the ballot:
| “ | A renewal of a tax for the benefit of the City of Maple Heights for the purpose of the payment of salaries for fire fighters in the Division of Fire and providing, maintaining, and operating fire apparatus, appliances, buildings and sites at a rate not exceeding 2 mills for each one dollar of valuation, which amounts to 20 cents for each one hundred dollars of valuation, for five years, commencing in 2013, first due in calendar year 2014.[1][2] | ” |
