A Mental Health and Recovery Services Tax Levy Renewal question was on the November 6, 2012 election ballot in Seneca County, which is in Ohio, where it was approved.

This question authorized the imposition of a 0.8 mill ($0.8 per $1,000 of assessed valuation) property tax levy for five years in order to fund the Mental Heath and Recovery Services of the District and county.[1]
