A Middleton Township Road Tax Levy Replacement question was on the November 6, 2012 election ballot in Columbiana County, which is in Ohio, where it was approved.

This question authorized the township of Middleton to impose a replacement property tax levy of 2 mills ($2 per $1,000 of assessed valuation) in order to fund road construction and maintenance.[1]
