A Pike Township Fire District Tax Levy question was on the November 6, 2012 election ballot in Stark County, which is in Ohio, where it was approved.
This question authorized the Township of Pike to renew a property tax levy of 1 mill ($1 per $1,000 of assessed valuation) for five years in order to fund the EMS and ambulance services of the township.[1]

Language on the ballot:
| “ | A renewal of a tax for the benefit of Pike Township Fire District for the purpose of providing and maintaining fire apparatus, appliances, buildings, or sites therefor, or sources of water supply and materials therefor, or the establishment and maintenance of lines of fire alarm telegraph, or the payment of firefighting companies or permanent, part-time, or volunteer firefighting, emergency medical service, administrative, or communications personnel to operate the same, including the payment of any employer contributions required for such personnel under Section 145.48 or 742.34 of the Revised Code, or the purchase of ambulance equipment, or the provision of ambulance, paramedic, or other emergency medical services operated by a fire department or firefighting company at a rate not exceeding one (1) mill for each one dollar of valuation, which amounts to ten cents ($0.10) for each one hundred dollars of valuation, for five (5) years commencing in 2012, first due in calendar year 2013.[1][2] | ” |
