The ballot title for Creation of a Local Urban Service Authority Amendment was as follows:

Shall the Constitution of Ohio be amended by adding Sections 5 and 6 to Article X to authorize the electors of municipalities, townships and counties to adopt by popular vote a charter creating an Urban Service Authority?

A 60% vote is required during one legislative session for the Ohio State Legislature to place a constitutional amendment on the ballot. That amounts to a minimum of 60 votes in the Ohio House of Representatives and 20 votes in the Ohio State Senate, assuming no vacancies. Amendments do not require the governor's signature to be referred to the ballot.
