A Middleton Township Road District Levy Addition measure was on the November 8, 2011 ballot in the Middleton township road district area which is in Wood County.

This measure sought to add to the current road district levy by a rate of 2 mills for a period of three years in order to further pay for road repair, construction and maintenance costs in the district.[2]
